WUSC Volunteer Contract
This form should be signed by the player's parent or guardian and submitted at time of registration, agreeing that if 4 hours of volunteer time are not performed by December 1, 2009, then their $50 deposit will be forfieted.
Scholarship Application WUSC Financial Assistance Form Use this form if applying for a scholarship. Please notify your team's coach if you are applying for financial assistance, and then send the form directly to the Club Registrar.
